not that anything would ever change anyone's mind on this, but it is definitely a slur for Italians, synonymous with guido. there is even an urban dictionary definition for it made in 2009.

He referenced himself as Fredo in an interview once. I get him being pissed for being approached like that in front of his family but then I also think, he just acted that way in front of his family.
of course, he isn't Italian so the reference was not the same

He can be 100% guaranteed he will be called Fredo the rest of his life. He's a thick dude, so if you call him it to his face, you better be ready for some action. Also looks like his roid rage is getting the best of him.
